Dental and Healthcare Plans Focus on Reimbursement for Periodontal Disease

Reimbursement for periodontal disease is vitally important to a dentist’s overhead. Probing for gum disease is extremely time consuming with measurements take at three point around every tooth on both sides of the tooth and those depths are recorded.

Roughly 55% of the entire U.S. population has dental coverage, according to NADP. Just over 70% of employers offer dental benefits, and 25% of employers that do not offer coverage are considering adding it next year. That’s great news for reimbusement for dentists when it comes to using the Lantis Laser!

What we are witnessing is an integration of medical and dental coverage. Some large health plans are making the case that integrated medical and dental coverage can lower overall health care costs.

You may have noticed Aetna, Inc. recently started running advertisements in major newspapers and magazines about the value of integration.

Aetna has a new dental/medical program that offers educational outreach about dental care, and enhanced dental benefits to members who are pregnant, as well as those who have diabetes, CAD or cerebrovascular disease.

Columbia University College of Dental Medicine, reviewed claims data on 145,000 Aetna members from 2001 and 2002. Among the individuals who had periodontal treatment earlier rather than later, the study found that:

Some dental plans are expanding product offerings for next year – including by covering more regular periodontal cleanings – in a bid to attract new members, improve oral health, and prevent more expensive medical problems down the road.

  • Members with diabetes had 9% lower medical costs (9% for risk adjusted, 8% for unadjusted);
  • Members with coronary artery disease (CAD) had 16% lower medical costs (16% for risk adjusted, 14% for unadjusted); and
  • Members with cardiovascular disease had 11% lower medical costs (11% for risk adjusted, 10% for unadjusted)

The enhanced benefit for pregnant women – who are enrolled in Aetna’s “Moms-to-Babies” program – include one additional cleaning visit; non-surgical removal of tartar, bacteria and rough surfaces; and follow-up visits that include spot removal of plaque and tartar to maintain a clean and healthy oral environment. The program for diabetes and CAD/cerebrovascular disease provides eligible members the same enhanced benefit as those for pregnant women, along with the addition of localized delivery of antimicrobial agents to treat periodontal disease.

CIGNA Dental Enhances Offerings

CIGNA Dental offers an expanded Oral Health Integration Program to include promoting the treatment of gum disease for members with diabetes and cardiovascular disease.

Under the program, CIGNA Dental members who are also enrolled in CIGNA HealthCare’s disease management programs for diabetes and cardiac care may receive 100% reimbursement for out-of-pocket costs associated with periodontal scaling and root planing – the non-surgical procedures to clean the roots of the teeth – and periodontal maintenance.

What CIGNA Dental is trying to do is remove the financial barriers to encourage treatment of those members who are potentially at risk, says Miles Hall, D.D.S., chief clinical director at CIGNA Dental.

By removing the financial barriers to oral health the company has saved medical costs.

Hall also says the integration of dental and medical is a strength of CIGNA, and is a selling point to attract new members.

According to CIGNA Dental, research shows that moderate or severe gum disease can increase the time a diabetic’s blood sugar remains high. Evidence also indicates that people with periodontal disease may be at higher risk for heart disease and stroke, the company noted. These two conditions are significant drivers of medical expenses.

Evelyn Ireland, executive director of the National Association of Dental Plans (NADP), says recent research has shown there is an 18% decrease in preterm births as a result of scaling and root planing for periodontal disease. Roughly half of the cost of births comes from premature births, she explains. Based on 2003 figures, an 18% reduction would result in roughly $3.2 billion in savings.

Oral Health Message Is Biting In

A study by The Guardian Life Insurance Company of America finds that 89% of Americans make the connection that their overall health is linked to their dental hygiene. Likewise, 85% of respondents in a new MetLife, Inc. survey said they believe there is a strong connection between oral health and overall medical health.
