Aggressive Treatment of Gingivitis Shows Significant Improvement in Arterial Elasticity

A recent study indicates patients given a deep-cleaning gum treatment, along with antibiotics, and the removal of, on average, two teeth which could no longer be treated successfully, experienced an important and statistically significant improvement in arterial elasticity at two months after treatment, with the effect being greater at six months.

The study enrolled 120 otherwise healthy patients with a severe form of periodontitis in a six-month trial and compared the effects of regular tooth cleaning with those of intensive periodontal treatment (scaling and root planing, locally administered antibiotics and tooth extraction, when necessary). The intensive treatment group showed important and statistically significant improvements in blood flow (as measured by brachial arterial dilation) within two months of treatment. The improvement was greater six months after treatment.

“In recent years, there have been numerous hypotheses linking periodontal infections to atherosclerosis – the process that leads to blockage of blood vessels,” notes Dr. Tonetti. “This study validates the effectiveness of intensive periodontal treatment in affecting vascular health.”

Currently, more than one in three Americans over age 30 have some form of periodontitis, according to the American Academy of Periodontology. Cardio Vascular Disease is the single largest killer of American adults; in 2003, it caused one of every five deaths in the United States. Unfortunately, periodontal infections often go untreated, creating a chronic inflammatory burden that may result in systemic inflammation, possibly contributing to broader health problems.

Lantis Laser with Optical Coherence Tomography (OCT) has exclusive rights to the application of OCT technology in the field of dentistry under its license Agreements with Lawrence Livermore National Laboratory and LightLab Imaging. OCT creates a noninvasive technique for imaging subsurface tissue structure with micrometer-scale resolution. Gum Disease Related to Cardiovascular Health

About 30 per cent of people worldwide are susceptible genetically to gum disease, according to the American Academy of Periodontology, based in Chicago. Gum disease has been linked to inflammation, a risk factor in heart disease, said Maurizio Tonetti, the lead author of the new study. Hardened blood vessels are associated with heart attacks and strokes.

Dr. Tonetti is a former professor of periodontology at the University of Connecticut, Farmington. “This is the first study that unequivocally establishes a link between gum infections and general health and well-being. ”

Tonetti and his team set out to find whether gum disease caused blood vessel dysfunction, and if an intensive treatment could change the early signs of hardened arteries.

A control group in the study received regular tooth cleaning and instructions in brushing and flossing. The researchers said they found no changes in the elasticity of these patients’ blood vessels.

Other patients were given a deep-cleaning treatment, along with antibiotics, and underwent removal of, on average, two teeth which could no longer be treated successfully, Tonetti said.

“In this group, we saw an important and statistically significant improvement in arterial elasticity already at two months after treatment, with the effect being greater at six months,” Tonetti said.

He suggested that people prevent gum disease by brushing and flossing, and by talking to their dentists about prevention and early treatment.

About three million people in the U.S. may have gum disease as severe as the cases in the study, according to the researchers.

Patients in the study experienced short-term inflammation immediately after initial treatment, according to the report.

The study published in the New England Journal of Medicine is promising for patients, the medical and dental communities and for health insurance carriers. Prevention is less expensive than treatment. This study adds to the growing body of evidence and underscores the potential impact of infection and subsequent inflammation on systemic diseases.

This is a perfect example of the studies that are mounting weekly which tie oral health to overall health. This body of evidence is the factor which has motivated health insurance carriers to begin to reimburse patients for issues related to oral health.
