VisualMED Clinical Solutions Corporation (OTCBB:VMCS) was established in 2004 with the sole purpose of marketing state-of-the-art Clinical Information Systems (CIS) to healthcare institutions, based on a robust core CPOE engine conceived by clinicians, integrated within a comprehensive patient electronic medical record.

One of the greatest challenges that will face health care providers over the next 10 years will be to improve patient safety in hospitals by the elimination of medical error.

In North America it is estimated that as many as 10% of patients admitted to hospital are subject to some form of medical error. Preventable medical errors are estimated to cause up to 100,000 deaths annually, with another 250,000 suffering some form of permanent injury. A significant number of these errors are medication errors, resulting in adverse drug events (ADEs), or are associated with other inadequate hospital procedures.

To combat this problem, a new generation of computer software has been developed, providing facilities for electronic prescribing, computer physician order entry (CPOE) and clinical decision support. Studies in live North American hospital environments indicate that the application of CPOE technology can reduce these types of errors by more than 50% – and, in some cases, by up to 86%.

As health care costs rise faster than can be sustained by regional and national governments, Health Departments and Ministries are recognizing that the long term solution will need to be based on better health care information management, management that can best be addressed by the implementation of state-of-the-art integrative solutions such as those marketed by VisualMED Clinical Solutions Corporation.

An emerging trend is developing in the government sector, with legislatures in France, Australia, Canada and several of the the United States developing consensus and legislation pushing forward an agenda that sees the rationalization of healthcare practice. This rationalization is accomplished by making available to clinicians information stored in heretofore disparate non-communicating information systems, while integrating the documentation of care into a uniform electronic medical record easily accessed by caregivers both locally and regionally.

CPOE

CPOE – a proven solution

In North America and Europe, a new generation of computer systems is fast becoming a major weapon in the battle against medical error. These systems are commonly classified as “Computer Physician Order Entry” or CPOE systems (although it is worth noting that many of these offer a wide range of decision support functions beyond the ordering and prescribing processes that the name would suggest).

CPOE systems are designed to trap potential errors at the earliest possible point in the treatment process. In many cases, this will be at the time that medication is ordered by the physician. Rather than writing a paper-based order, the physician enters the order into a computer, which then checks it for incorrect dosages, incorrect drugs, drug-allergy interactions, drug-drug interactions, drug-food interactions and other possible causes of error. Further along the path, the system can then monitor the treatment process, ensuring that nursing staff are administering the correct drug to the correct patient at the correct time, and can issue an alert if the patient’s condition has changed in such a way that a new course of treatment should be considered.

CPOE systems are not intended to take the decision-making process away from the medical practitioners, but are designed to provide the practitioner with the necessary data to make informed decisions.

CPOE – improving safety and reducing costs

Studies of live CPOE systems in North American hospitals have indicated that CPOE can have a dramatic effect in reducing the incidence of errors. In several cases, error rates have been shown to be reduced by 55% to 70% and serious medication errors by up to 86%.

In addition to the immediate effect on error rates, CPOE has also been found to lower stress levels significantly among medical and nursing staff, to improve decision-making capabilities, and to enhance training facilities – all of which can be expected to help create a safer working environment.

Significant reductions in error rates also lead to substantial cost savings. As an example, in one hospital the average length of stay was reduced by 0.9 days after the introduction of CPOE, and hospital charges were reduced by 13%.

The VisualMED System

The VisualMED Clinical Information System (CIS) is a scaleable enterprise-wide Electronic Medical Record with CPOE decision support at its core whose development is based on 10 years of experience with our first generation physician order entry system, still in continuous use at a tertiary care university-affiliated healthcare institution. The VisualMED Clinical System seamlessly integrates physician order entry and alerts / decision support with nursing clinical functions, including an automatically generated medication administration record (MAR), as well as with doctors’ and nurses’ notes. Given the need to access clinical and laboratory data for the purpose of decision support, results reporting and clinical data entry are integrated as well. Designed by clinicians with more than 20 years hands-on experience in the tertiary care environment, the VisualMED Clinical System is modular in concept and responds to the unique needs of each of the medical and surgical specialties.

The VisualMED Clinical System is compatible with existing legacy systems and communicates with them through HL7 interfaces. User-defined data fields can be created, and the system allows ad hoc generation of requisition forms similar in appearance to an institution’s existing requisitions for those services not on-line. Data can be accessed off-site in secure, read-only mode through VisualMED-Web, using Citrix Server for secure, encrypted, off-site data transmission.. VisualMED security uses biometric logon and order sign-off throughout, clearly superior to traditional ID/Password systems. All data access is audited, and our software adheres to HIPAA proposed regulations of September 2000. Our legacy product was reviewed by Health Canada staff in 1995, and formally approved for medication and narcotic medication prescription. The system’s integrated DICOM viewer allows users to review image test results in the context of clinical patient findings, is interface ready, and is compatible with existing commercial PACS.

New Implementations

The Segal Cancer Center of the Mortimer B. Davis Jewish General Hospital of Montreal is implementing VisualONCOLOGY, a state-of-the-art interactive, interdisciplinary electronic medical record by VisualMED Clinical Solutions Corp. The site, part of the McGill University teaching hospital system, will use VisualONCOLOGY for documentation of patient care, decision support and for the design and execution of protocol chemotherapy. Implementation is starting immediately with an anticipated go-live as early as Q4 of this year. The Cancer Center will take advantage of VisualONCOLOGY’s fully electronic clinical documentation to free additional space for patient care, while VisualONCOLOGY’s cohort-identification and protocol management capabilities will contribute directly to clinical research.
